JUVÉDERM VOLBELLA® XC

JUVÉDERM VOLBELLA® XC is the latest U.S. Food and Drug Administration (FDA) approved injectable hyaluronic acid (HA) filler from Allergan that is formulated for subtle enhancement of the lips and to treat the etched wrinkles around the mouth. It was found to be effective for up to 12 months in the clinical trials that were performed and has been widely used in Europe since 2011.

The HA in both JUVÉDERM VOLBELLA® XC and the previously approved filler for cheek enhancement, JUVÉDERM VOLUMA® XC, was developed with advanced Vycross® technology to produce smooth, long lasting results. Vycross® technology enables treatments to be performed with a lower concentration of HA resulting in less swelling. Dr. Forley will evaluate you during your consultation to see if you are a good candidate for JUVÉDERM VOLBELLA® XC.

JUVÉDERM VOLUMA™ XC

Volume loss in the cheek is a common aspect of aging that can be successfully treated with injectable fillers. Improving the projection and contour of the cheek creates a more youthful appearance that  postpones the need for more invasive surgical techniques.

JUVÉDERM VOLUMA XC was FDA-approved in October 2013 for restoring volume to the cheek. It is an injectable gel and is composed of a smooth yet dense hyaluronic acid (HA) that is suitable for deep injection. Similar to its companion product JUVÉDERM® XC, placement of the filler is performed with a series of injections that are targeted towards producing the desired contour.  HA is a natural component of your skin that diminishes with age resulting in decreased skin structure and volume. The flattened or hollow appearance of aging can be converted to a subtle, uplifted look that can be customized with JUVÉDERM VOLUMA XC to balance the aesthetics of your face. It has been proven in clinical studies to last up to 2 years.

COSMETIC PROCEDURES FOR MEN

Men have become an increasingly common presence in the plastic surgeon’s office. Competition in the workplace and the prevailing youth culture have combined to create a desire for self-improvement that may not have seemed as important 15 or 20 years ago. The minimal downtime procedures performed today make their acceptance amongst busy professional men and women very high. The most frequent non-surgical procedures men are having done in our office are BOTOX® Cosmetic, injectable fillers, and Ultherapy®. Surgery often is performed to treat baggy eyelids, excess neck, hip, and tummy fat, and gynecomastia or male breast enlargement.

NON-SURGICAL

BOTOX® Cosmetic

Botox® Cosmetic provides a non-surgical way of reversing the crow’s feet, laugh lines, and frown lines produced by the underlying contraction of facial muscles. “Also known as Bro-tox, Botox is one of the most popular non-surgical cosmetic treatments for men” say Philadelphia dermatologist, Dr. Ringpfeil. It works by enabling a smooth skin appearance as a result of blocking the muscle contraction and overlying skin wrinkling produced by nerve impulses. The untreated muscles will allow for normal facial expression to remain unaffected. The treatment lasts from 4-6 months and is performed 2-3 times per year to maintain youthful appearing skin.

Injectable Fillers

Injectable dermal fillers composed of hyaluronic acid, such as JuvedermUltra and Ultra Plus, Restylane®, and Perlane® are indicated in the treatment of folds, wrinkles, or hollowing that results from facial aging. “By adding volume, wrinkles and folds appear smoother and facial contours appear more youthful” says Dr. Forely. The duration of effect is up to 12 months.

Another filler we offer is Sculptra® Aesthetic. This product is composed of poly-L-lactic acid (PLLA), an injectable biocompatible material that stimulates the production of the skin’s own collagen. The natural appearing restoration of youthful facial contours is made possible by the skin thickening that results from the gradual deposition of the new collagen. Generally a series of 2-3 treatments at 6-week intervals are necessary to achieve the desired effect. The youthful appearance that results from the treatment can last up to 3 years at which time maintenance treatments can be utilized.

Ultherapy®

Ultherapy® is a new type of non-surgical treatment that uses ultrasound energy to safely lift and tone the skin without any downtime. The ultrasound image on the viewing screen allows Dr. Forley to see precisely the level at which the ultrasound energy is being delivered. Ultherapy® apparatThe deep layers of the face and neck that have lost their elasticity and firmness over time are targeted by the heat-producing sound waves. Although one treatment is frequently all that is needed, more significant facial and neck sagging may require an additional treatment after 3-6 months. The duration of results can vary but you can generally expect up to 2 years of benefits from Ultherapy®.

Loose, sagging, bulging tissues of the eyelids can make the eye appear smaller and lead to others asking if you are tired or stressed. Blepharoplasty or eyelid surgery removes excess loose skin and fatty tissue that produces sagging and heaviness of the upper or lower eyelids. The eye appears larger, less tired, more open, and brighter.

Liposuction is a contouring procedure that removes bulges to achieve a more aesthetic blending of body outlines. Excessively large male breasts, known as gynecomastia, can produce a feeling of self-consciousness about body image that can inhibit lifestyle choices and daily activities. In most cases, the glandular and fatty tissue enlargement of the male breasts begins during puberty. It is usually a benign condition, but can be associated with hormonal abnormalities that may need additional evaluation. Gynecomastia procedures improve the large breast size by reducing the volume and prominence of the breast fatty tissue using liposuction and directly removing excess glandular tissue via an incision at the lower edge of the areola. Skin excision is rarely necessary as it will generally retract during the healing phase. In addition to the improved appearance, patients also find they are comfortable participating in social or sports activities that they were either unable or unwilling to do before surgery. During your consultation Dr. Forley will discuss and recommend those options that he feels will give you the best result.

BOTOX® COSMETIC & INJECTABLE FILLERS: WHAT, WHEN, AND WHERE

We often are asked to clarify the reason for choosing BOTOX® Cosmetic versus injectable fillers, where each one works best, and when is the right time to seek treatment.

Frown lines, forehead wrinkles, and laugh lines, the end result of years of repetitive contraction of our facial muscles, can be selectively modified with tiny injections of BOTOX® Cosmetic. The decrease in the intensity of muscle contraction that results from BOTOX® Cosmetic treatment allows for a progressive smoothing of the overlying wrinkled skin that lasts for 4-6 months. Early intervention at the onset of these wrinkles will not only improve your current appearance but prevent a steady deepening of the lines, which makes them more difficult to treat. “The key to maintaining consistently smooth skin is to re-treat the areas when the effect on the muscle wears off so that the wrinkles don’t recur with renewed muscle activity,” says Dr. Forley. As the cheeks start to sag, folds and jowls appear in the face, adding to the sense that we are visibly aging. In our previous blog, “Why Volume Matters”, we discussed the importance of volume in the face and the effect that decreased or displaced facial fat has on the appearance of aging. Flattening of the cheek, deepening of the nasolabial fold, and disruption of  jawline definition by the jowl are all signals that we are getting older. Sculptra®Aesthetic, composed of poly-L-lactic acid, is an additional injectable treatment option for aging changes in the face. Restoration of youthful facial contours will occur more gradually with Sculptra®Aesthetic because it works by stimulating the production of collagen.  Added volume emerges over a 3 month period as a result of a thickening of the collagen layer of the skin in contrast to the immediate effect produced by the hyaluronic acid fillers just described. Generally, a series of 2-4 treatments at 6-week intervals are necessary to achieve the desired outcome with results lasting up to 3 years.

AM I READY FOR A FACELIFT?

Everyone wants to know what age is appropriate to have a facelift – but there is no ideal age or time frame.

The best answer is when you feel you are ready. There is no specific age range because each person is unique in so many ways.  Environmental, genetic and hormonal factors influence the aging process and each individual responds differently. A facelift may be right for you when you begin to experience the effects of gravity – as the aging process takes hold of facial tissues causing your neck to loosen up, jawline to slacken and deepening of the nose-to-mouth folds.  Typically women tend to come in for consultations in their mid 40s when the face they see in the mirror every morning seems to be changing. Men tend to wait until their 50s when neck sagging starts to become a concern.

The goal of a facelift is to make you look as good as possible for your age with a natural, unoperated appearance.  It is important to understand what a facelift can and cannot achieve.  A facelift will not stop the hands of time, but it can reset the clock about 5-10 years. By recontouring the neckline, we can create a smooth jawline and restore a youthful cheek position. A short-scar lift discretely camouflages scars within your hairline at the temples, around the curvature of the ear, and in some cases within the edge of the ear.  If you have more excess skin, the incisions may need to be extended behind your ear as part of classic SMAS-facelift technique, but they will be carefully closed to produce fine scars.  In either case, you will be able to wear your hair as you normally do, even in an updo, without feeling self-conscious.

While the facelift will rejuvenate the lower third of your face and neck, but it does not address your eyes or forehead.  Eyelid surgery can be combined with a facelift. The brow area can be lifted with BOTOX® Cosmetic or Dysport, as well as an endoscopic browlift if necessary.

Coupling a facelift with fillers enhances and maintains the results creating a balanced youthful appearance.  Juvederm™ can be used for deeper folds and augmenting the lips, while fat may be used at the time of the facelift to plump areas that need volume restoration, such as the cheeks or hollow lower eyelids.

SILHOUETTE LIFT: BRIDGING THE GAP BETWEEN INJECTABLES & SURGERY

If you are in your 40s and have been going for injectable fillers like Juvederm, Radiesse and Perlane, you may feel like you reach a plateau at some point. It may be time to ask about the next steps in facial rejuvenation.  Until recently, the next step after using injectable fillers was a facelift due to limitations of adding volume alone. Injectable fillers mask the signs of aging by adding volume to sagging facial tissues helping to camouflage flattening of the cheeks, deepening of the nasolabial folds, and the loss of a smooth jawline contour.  However, as facial tissues continue to descend, a larger amount of filler is needed to camouflage the appearances of aging.

The Silhouette-Lift is a minimally invasive procedure, which effectively bridges the gap between injectables and surgery by lifting sagging facial tissues away from the mouth and jawline, restoring a youthful fullness to the cheek.  The procedure is an excellent alternative if you are not ready to commit to a facelift, but are seeking aesthetic improvements. The Silhouette Lift utilizes specially modified polypropylene sutures with bioabsorbable cones approved by the FDA.  The sutures are introduced through a small incision within the hair of the temporal scalp.  Four sutures on each side of the face are passed through the superficial tissue under the skin.  The unique threads grasp and elevate sagging facial tissues with multiple fixation points provided by the cones and are secured to the temporal fascia.  The body absorbs the sutures by producing fibrous tissue around the cones, which stabilizes and maintains the lift.  The entire procedure can be preformed in an hour using local anesthetic.
